Emphasizing thinking about the risk to Peter, as well as the likelihood of needing the gun for protection, seems to have prompted Ms. Fernandez to consider various options. You call Peter back into the room and revisit the idea of him visiting with a doctor who specializes in mental health. His mother is open to the idea and Peter does not object. You give them the referral information and suggest that they call for an appointment upon returning home. However, if Peter seems at immediate risk of self-harm, make the call and appointment from your office before they leave. Make sure you have access to a number of immediate options or resources. Knowing some teen friendly mental health care providers located at your office, hospital and close to home is essential.

  • Prior to leaving the exam room, you give Ms. Fernandez and Peter one last opportunity for questions.
  • You schedule Peter back for a follow-up visit in one month. In addition, you call Ms. Fernandez 24 hours later to ask about the referral and her plans regarding firearm storage, and you encourage Ms. Fernandez to bring Jack’s father along for the next visit.
  • You give Ms. Fernandez information on safe storage to parent handout/educational materials on firearm safety. You encourage her to think about what you have discussed.
